RN Care Coordinator

About the role

The RN Care Coordinator works collaboratively with physicians and clinical staff to support patients who have chronic conditions or complex health needs. The role follows established clinical guidelines to ensure high‑quality patient care.

Key responsibilities

• Partner with physicians and multidisciplinary teams to coordinate patient care plans.
• Monitor and manage patients with chronic or complex conditions according to clinical guidelines.
• Facilitate communication between patients, families, and healthcare providers.
• Ensure documentation is accurate and up‑to‑date in the clinic’s records.

Required profile

• Current RN license in good standing (Arkansas or multi‑state including Arkansas).
• CPR certification.
• High School diploma or GED equivalent (required for lab work).
• Three to four years of nursing experience preferred.
• Experience in ambulatory care or care management preferred.
• Valid driver’s license with reliable, insured transportation.
