About the role
The RN Care Coordinator will join the Woodside Medical Clinic team to provide coordinated nursing support for patients with chronic conditions or complex health needs. Working closely with physicians and clinical staff, the role ensures care is delivered in line with established clinical guidelines.
Key responsibilities
- Collaborate with physicians and clinical staff to support patients with chronic conditions and/or complex needs according to established guidelines.
Required profile
- Current RN license in good standing (Arkansas license or multi‑state license that includes Arkansas).
- Current CPR certification.
- High School diploma or GED equivalent (required for lab work).
- Three to four years of nursing experience preferred.
- Ambulatory and/or care‑management experience preferred.
- Valid driver’s license with reliable, insured transportation.
- Availability to work day shift, Monday‑Friday 8 AM‑5 PM, 80 hours bi‑weekly.
